Kindergarten Round Up 

We are excited to offer our rising Kindergarten students with the opportunity to participate in our annual Kindergarten Round Up. Participation in the Kindergarten Round Up will allow us to get to know your child and will prepare us to better serve your child during the 2017-2018 school year. Please plan to bring your child with you when you register so that he/she can be introduced to your new Wynbrooke Family. Students' attendance is MANDATORY.

Students returning to the same school 
 
In order to continue enrollment, parents must provide a current proof of residence as well as update the DCSD registration form, media release, and any school-related materials each year (Fernbank field trip permission, dismissal arrangements, clinic forms, etc.) 
 
Who can register a student? 
  • A child may be registered by his or her
    • parent; or
    • Legal guardian, including an agent grandparent, as provided by the “Power of Attorney for the Care of a Minor Child Act,” O.C.G.A. § 19-9-120 et seq;
    • Guardian with a special or military power of attorney, relative to the guardianship of a child of a military family, 10 U.S.C. § 1044b; or
  • An adult at least 18 years of age or an emancipated minor at least sixteen years of age residing within the boundaries of DCSD, who is not the parent or guardian of a child, but assumes the duties and responsibilities of a parent and has authority to enroll the child in a publicly-funded Georgia school;
  • The Department of Human Services (DHS) or the Department of Juvenile Justice (DJJ) or the custodian of a student placed by DHS or DJJ in a residential facility located within DCSD, as provided for in O.C.G.A. §20-2-133(b); or
  • A child who is, or whose parent or spouse is, a migratory agricultural worker, including a migratory dairy worker, or a migratory fisher, and who, in the preceding 36 months, in order to obtain, or accompany such parent or spouse, in order to obtain, temporary or seasonal employment in agricultural or fishing work-(i) has moved from one school district to another; or (ii) resides in a school district of more than 15,000 square miles, and migrates a distance of 20 miles or more to a temporary residence to engage in a fishing activity, 20 U.S.C. § 6391 et seq.
 
 
 
Students new to the District must present the following:
 
Kindergarten
 
Evidence of the student's date of birth: 
  • an original or state-certified copy of the child's birth certificate or a certified hospital issued birth record; 
  • a military ID; 
  • a valid driver's license; 
  • a passport; 
  • an adoption record; 
  • a religious record signed by an authorized religious official; 
  • an official school transcript; 
  • or if none of the other documents can be provided, an affidavit of age sworn to by the parent, guardian, grandparent, or other person, accompanied by a certificate of age signed by a practicing physician, which certificate states that the physician has examined the child and believes that the age as stated in the affidavit is substantially correct.
  • The child must be five (5) years old on or before September 1, 2016 or a child who was a legal resident of one or more other states or countries for a period of two years immediately prior to moving to Georgia and who was legally enrolled in a public kindergarten, or a kindergarten accredited by a state or regional association or the equivalent, must be eligible for enrollment in the appropriate education program if the child attains the age of five for kindergarten by December 31, 2015 and is otherwise qualified.
  • Social Security Card, or waiver pursuant to O.C.G.A. § 20-2-150(d);
  • Certificate of Nutritional Screening and Eye, Ear, and Dental Examination (GA form 3300 - GA-DHR) O.C.G.A. § 20-2-770;
  • Certificate of Immunization (GA form 3231 - GA-DHR), waiver, or affidavit of religious conflict; however, immunization may be required in cases when a disease is in epidemic stages pursuant to O.C.G.A. § 20-2-771;
  • Photo ID of the parent or guardian enrolling the child;
  • Proof of Residence documents; and
  • Proof of custody or guardianship if not the birth parent. In the case of agent grandparents, as per O.C.G.A. § 19-9-120 et seq., a copy of the Power of Attorney document that is in the substantially same form as provided in O.C.G.A. § 19-9-129(b)-(c), and proof of the parent's stated hardship.
First Grade
 
Evidence of the student's date of birth: 
  • an original or state-certified copy of the child's birth certificate or a certified hospital issued birth record; 
  • a military ID; 
  • a valid driver's license; 
  • a passport; an adoption record; 
  • a religious record signed by an authorized religious official; 
  • an official school transcript; 
  • or if none of the other documents can be provided, an affidavit of age sworn to by the parent, guardian, grandparent, or other person, accompanied by a certificate of age signed by a practicing physician, which certificate states that the physician has examined the child and believes that the age as stated in the affidavit is substantially correct.
  • The child must be six (6) years old on or before September 1, 2016 or a child who was a legal resident of one or more other states or countries for a period of two years immediately prior to moving to Georgia and who was legally enrolled in a public first grade, or a first grade accredited by a state or regional association or the equivalent, must be eligible for enrollment in the appropriate education program if the child attains the age six for first grade by December 31, 2015 and is otherwise qualified.
  • Social Security Card, or waiver as per O.C.G.A. § 20-2-150(d);
  • Certificate of Nutritional Screening and Eye, Ear, and Dental Examination (GA form 3300 - GA-DHR) O.C.G.A. § 20-2-770;
  • Certificate of Immunization (GA form 3231 - GA-DHR), waiver, or affidavit of religious conflict; however, immunization may be required in cases when a disease is in epidemic stages O.C.G.A. § 20-2-771;
  • Photo ID of the parent or guardian enrolling the child;
  • Proof of Residence documents; and
  • Proof of custody or guardianship if not the birth parent. In the case of grandparent agents, as per O.C.G.A. § 19-9-120 et seq., a copy of the Power of Attorney document that is in the substantially same form as provided in O.C.G.A. § 19-9-129(b)-(c), and proof of the parent's stated hardship.
All Other Grades
  • Students who have not attained the age of 21 by September 1, 2016 or have not received a high school diploma or the equivalent are eligible for enrollment. Students that have dropped out of school for one quarter or more are eligible to enroll in the appropriate general education programs unless they attain the age of 20 by September 1 as prescribed in O.C.G.A. § 20-2-150;
  • Special education students shall be eligible for enrollment in appropriate education programs through age 21 or until they receive high school or special education diplomas or the equivalent; provided, however, they were enrolled during the proceeding school year and had an approved Individualized Education Program (IEP) which indicated that a successive year of enrollment was needed.

  • Photo ID of the parent or guardian enrolling the child;
  • Proof of Residence documents;
  • Discipline report (middle and high schools);
  • Withdrawal records from previous school including last grade placement; and
  • Proof of custody or guardianship if not the birth parent. In the case of agent grandparents, as per O.C.G.A. § 19-9-120 et seq., a copy of the Power of Attorney document that is in the substantially same form as provided in O.C.G.A. § 19-9-129(b)-(c), and proof of the parent's stated hardship.
Note: Immigrant Status
  • Schools shall enroll immigrants/non–visa holders meeting age and residency requirements and shall not inquire about their legal status.
Proof of Residency

  • current utility bill (gas, electric, or water only – no phone bills);
  • Signed lease of residence; or
  • Signed home mortgage contract.
Event of Co-Residency

  • The Affidavit of Residence must be completed by the parent(s)/legal guardian(s) and homeowner/lessee and submitted to the school, with all required documents attached.
  • The Affidavit of Residence must be submitted before the start of school or upon moving into the attendance area of the new school.
  • The parent(s)/legal guardian(s) and the owner/lessee must both be present to submit the Affidavit of Residence and submit the required documents to the school (no exceptions will be made).
  • The Affidavit of Residence must be updated annually by both the homeowner/leaseholder and the parent(s)/legal guardian(s).
